CLAY Lewi Siosi

The American Association for Hand Surgery (AAHS) and the Hand Surgery Endowment (HSE) established the International Reverse Fellowship program in 2017. The program sponsors an international hand surgeon’s visits with AAHS member surgeons at their institutions with the aim to provide expert, hands-on experience so that the Fellow may return to his or her host country and better local care while also educating others in learned techniques to perpetuate the improvement of global hand care.

During my tenure as an International reverse Fellow i worked with various Hand Surgeons in USA:

• San Diego, California (Lorenzo PACELLI MD)
• OrthoCarolina Hand, North Carolina (Glenn GASTON MD)
• Children's Hospital, Houston Texas (Christopher William PEDERSON MD).

• Perform diagnostic evaluations and develop treatment plans for patients with trauma, orthopedic and hand injuries and conditions
• Conduct orthopedic and Hand surgery procedures to repair or reconstruct orthopedic and hand injuries and conditions
• Provide post-operative care, including wound care, rehabilitation and follow-up appointments
• Collaborate with other medical professionals including physical therapists and nurses, to ensure optimal patient outcomes.
• Stay current with advancements in hand and orthopedic surgery and incorporate new techniques into practice
• Maintain accurate medical records and document patient progress
• Communicate effectively with patients, families, and other healthcare professionals
• Work as leader of Hand Team for the management of hand and wrist injuries.
• Dedicated microsurgery on-calls including paediatric hand trauma and covering hand and wrist injuries at National Referral Hospital Solomon Islands.
• As hand team leader i am part of the general orthopaedic on call rota.
• Electively, I am also managing maxillofacial trauma and malignancy surgery the dental/oral surgery team managing both local referrals and tertiary referrals in Solomon Islands.

The Loloma Foundation’s mission is to provide sustainable medical, dental and infrastructure support to rural communities in the South Pacific who would otherwise have no access to basic healthcare.
The Loloma Foundation is a California-based 501(c)(3) non-profit corporation, established in 2001. The board members consist of six medical, dental and business professionals who share a common goal of improving the lives and health of the people of the South Pacific.

I traveled annually to Taveuni and Savusavu district hospitals, Fiji Islands, volunteering as a Hands/Plastics surgery team member and trainee, Loloma Foundation Medical Missions.

CLINICAL

• Provides orthopaedic inpatient and outpatient care in National Referral Hospital in accordance with departmental needs and objectives including procedures and operations according to credentialing and scope of practice.
• Provides clinical oversight, support and advice to medical practitioners in regard to orthopaedic patients.
• Ensures patient medical records are kept according to health service protocols and professional standards.
• Ensures discharge summaries and medical reports on inpatients that have been under their care are prepared promptly.
• Participate in after hours on-call roster.

EDUCATION

• Promotes the development of training in medicine in country health.
• This would entail the direct supervision of medical students and residential medical officer trainees in orthopaedics, as well as assisting gaining vocational training positions in orthopaedics.
• Maintain knowledge and expertise regarding developments in orthopaedic surgery.
• enrolled in Pacific Islands Orthopedic Association training Programme.

ADMINISTRATION AND MANAGEMENT

• Participates in quality improvement programs and clinical governance activities applicable to orthopaedic surgery.
• Participates departmental and National referral Hospital committees as required.
• Attends to medico legal issues that arise concerning patients that have been under my care and advises the Medical Director about complaints received pertaining to themselves or other doctors within my department.
• Participates as an executive member of Solomon Islands Medical Association, ensuring that Scheme of service for all medical doctors delivered.

Primary Purpose:

Responsible for the day to day management of private and public inpatients and outpatients within the allocated site/facility. Provide after-hours emergency as determined by the relevant work unit.

Duties:

• Admission and care of patients and liaison with Specialist Medical Staff regarding patients.
• Attend Specialist ward rounds and record all decisions made in the medical history. Timely response to calls by members of the patient’s care team.
• Ensure that consultations occur when requested.
• Interact with families or carers of patients. Accurate and timely recording of drugs and treatment.
• Undertake procedures as required.
• Accurate, comprehensive and contemporaneous recording in the medical record including progress notes each day.
• Discharge planning. Provide Discharge Summary compliant with policy (or the appropriate standards) to ensure safe clinical handover to community-based care providers.
• Provide after-hours cover as directed and short-term relief in other areas as required by the employer.
• Involvement in Quality Assurance activities. Involvement in educational activities or evidence of further self-directed learning as per registration requirements.
• Supervision of Residents, Interns and Medical Students.
• Actively participate in and contribute to the organisation’s Quality & Safety and Work Health & Safety processes, including in the development and implementation of safety systems, improvement initiatives, safeguarding practices for vulnerable people, and related training.

Key Accountabilities and Responsibilities:

The Registrar/Senior Registrar works under the direction and supervision of Specialist Medical Staff, and is responsible for:

• Adherence to professional protocols, policies, clinical pathways and standards.
• Demonstrating sound judgement and competence in accordance with skills and knowledge when undertaking tasks.
• Ensuring work is carried out in accordance with relevant Work Health and Safety legislation and procedures.
• Where applicable, exercise delegations in accordance with a range of Acts, Regulations, Awards, administrative authorities and functional arrangements as mandated by Statutory office holders including the Secretary and Head of State Service. The relevant Unit Manager can provide details to the occupant of delegations applicable to this position.
• Comply at all times with policy and protocol requirements, including those relating to mandatory education, training and assessment.

This role oversees the daily management of medical services in Malaita Province and is responsible for directing curative services by providing leadership in clinical governance, professional affairs, continuous quality improvement, and patient care and safety.

Provincial Health Director is the accounting officer for provincial health Department.

• As Primary Health Care Medical officer sole responsibility is to provide overall guidance and champion the primary and preventive medicine in rural communities and villages.
• The primary health care doctor is also responsible for investigation of epidemics or disease outbreaks in population or communities.
• Perform primary health care programs to remote communities/islands.
• Responsible to lead vaccination, health statistics program, Maternal health, TB/Leprosy and HIV/Aids programs.
• Promote health education in rural communities.
• Provide assessments/feedbacks on safety of rural health facilities.
• collaborate with community health consultants (nurse Practitioners) on daily patient care.
• liaise with communities to resolve issues affecting rural health facilities and workers.
• Advocates for health islands and village setting programs.

To coordinate and document the admission, management and discharge planning of patients under the supervision and direction of Registrars and Senior Medical Staff. This will include:

• Ensuring continuity of care for patients, including participation in clinical handover at the beginning and end of each shift.
• Rotation through various clinical units.
• Undertaking rostered duties after hours (including weekends), which may require the provision of cover to more than one unit.
• Participation in an on-call roster and being available for duty as required.
• Comply with Quality Management policies and procedures and participate in Quality Management activities.
• Assist in maintaining a safe work environment by reporting any incidents of unsafe work practices, sites/areas or equipment.
• Collaborate with team members to fulfill and maintain bell-wealter status/role of Kiluufi Provincial hospital.
